Complete the form, filling in every blank.
1. Your ID number is determined by using your two initials (first and last name) and the last four digits of your phone number. (Example: AB-1234)
2. You must also put the name and ID number of the person that introduced you.
3. Make sure to sign and date.
4. Any missing information will cause a delay in the processing of your application.
5. When your application is received and processed, you will be e-mailed a temporary password to log-in to the Subscriber Area.


Send your completed application with your CASH payment to The Technical Center address at the bottom of the application using one of the following methods:
1. Priority Mail with a U. S. Postal Service Delivery Confirmation Receipt. (This is usually two-day delivery)
2. First Class Mail with “Certified Delivery” (This cost less, but may take longer for delivery)

For either option, you can access the postal internet website, which appears on your receipt, to confirm delivery.
